First Eye Care Services provides Individual Residential Services and Supports for adults with intellectual and developmental disabilities across Colorado. Not a facility. A home, with no more than three people in it, and staff who learn who you are.

The mission of First Eye Care Services is to assist persons with intellectual and developmental disabilities to be healthy, safe and active members of their community. Love, patience, and compassion are the embodiment of the care we provide.
What we do
Individual Residential Services and Supports
IRSS is residential care delivered in a real home rather than an institution. In a host home, a trained Host Home Provider shares their home with no more than three individuals. Support can also be delivered in a home owned or leased by the agency, or through a Personal Care Alternative where staff rotate in shifts.
Which setting fits depends entirely on the person. How much support they need, how they want to live, and who they want to live with are all part of the conversation, not decisions made for them.
Three ways we deliver IRSS
- Host home. A seasoned Host Home Provider shares their home with up to three individuals.
- Agency home. A home owned or leased by First Eye Care Services.
- Personal Care Alternative. A home where our employees rotate in shifts around the clock.
Frequency and intensity of support are set by each person's service plan, never by a standard package.
Why we exist
Founded by a father
First Eye Care Services was founded by Paul Osei Afriyie, a pastor whose son Aaron has level 3 autism. When Aaron reached the end of his schooling, Paul faced the question every family in that position eventually faces. What happens now?
Rather than accept the usual answers, he built somewhere for people with significant disabilities to find their place in the world, and to build a community with one another.
